Job Description:Position PurposeThe Care Management Support Coordinator II is responsible for supporting administrative care management activities, including outreach, inbound call support, and scheduling services. This role serves as a key contact for members, providers, and staff to resolve issues, coordinate services, and document member interactions in accordance with state and regulatory guidelines.
Primary Responsibilities
- Conduct outreach to members to assist with care plan next steps, answer questions, and provide ongoing education to both members and providers.
- Serve as the first point of contact for inquiries, requests, or concerns from members and providers.
- Support members by connecting them with health plan and community resources to ensure a high level of care and service.
- Apply knowledge of health plan activities and available resources to assist members effectively.
- Assist with onboarding by sending welcome letters, correspondence, and educational materials to foster positive member and provider relationships.
- Document and maintain non-clinical member records, ensuring compliance with state and regulatory requirements.
- Make referrals to address Social Determinants of Health (SDOH) needs by leveraging knowledge of local benefits and resources.
- Perform administrative duties such as scheduling services, explaining care plan procedures, and sending program updates.
- Comply with all organizational policies, standards, and procedures.
- Perform other duties as assigned.
QualificationsEducation and Experience:
- High school diploma or GED required.
- 1-2 years of experience in a related field.
- Experience in a managed health plan, call center, or medical facility is a plus.
Required Skills:
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Strong spoken communication skills and professional phone etiquette.
- Excellent organizational and time management abilities.
- Ability to type accurately and efficiently.
Preferred Skills:
- Previous call center experience.
- Familiarity with healthcare, care management, or member services in a health plan or medical facility.
Performance Expectations
- Ensure timely and accurate completion of administrative tasks.
- Maintain high-quality communication and customer service standards.
- Accurately document member interactions in alignment with state and regulatory requirements.
- Contribute to team goals and uphold organizational values.
