Work Location : UHealth TowerThe University of Miami/UHealth has an exciting opportunity for an RN Case Manager in the Case Management department at the UTower location.
This position functions as a RN Case Manager for the Case Management Department under the direction of the Director of Case Management.
Incorporates the fundamental principles of utilization management, monitoring resource consumption, social services intervention and individual discharge planning that will include assuring that the transfer or discharge of a patient to another level of care, treatment, services or different setting is always based on the patient's assessed needs and the organizations capabilities to meet these needs.
Facilitates delivery of clinical and community services to patients and families through effective utilization of available resources.
Manages the observation patients on their assigned unit, conduct InterQual® reviews for admission, and continued stay per policy, report avoidable days and facilitate clinical reviews to third party payors
Ensures the appropriateness and cost effectiveness of patient's plan of care. Report all complex cases to the Director and/or Care Coordination Manager as often as necessary.
Proactively collaborates with physicians(s) to develop patient care plans and review for medical necessity for continued hospital services and resource consumption.
MINIMUM QUALIFICATIONS:
BSN graduate of an accredited school of nursing.
Valid State of Florida RN license.
Minimum of one (1) year in Hospital Case Management/nursing.
Working knowledge of patient assessment, medical terminology.
Current BLS - CPR Certification.
