JOB RESPONSIBILITIES

• Assist with review of transition resources and services.
  
• Conduct community outreach and family/youth engagement activities.
  
• Develop and maintain strong relationships with family caregivers and youth with Autism for a successful transition in the community.
  
• Develop and utilize transition tools, materials, and resources.
  
• Conduct family learning sessions and workshops to facilitate a successful transition.
  
• Provide one-on-one support to families/youth with autism to support a successful transition.
  
• Attend partner and advisory council meetings.
  
• Assist with data collection.
  
• Disseminate information and resources.
  

JOB QUALIFICATIONS

• Bachelor's degree in a related field
  
• Interested in/knowledgeable about Autism/developmental disabilities across the lifespan
  
• Part-time availability (16-20 hrs/week) mostly daytime hours (Mon-Fri); some evenings and weekends; Reliable transportation a must; Hybrid schedule possible; some in-person, on-site and significant community-based work required
